What We Offer
We Formed a Coalition to Improve the Quality of End of Life Care
Through Education and Also to Provide Information on Decision Making,
Local Resources for End-of Life, and Training Programs .
How We Serve
The Coalition Serves Through a Website, Personal Telephone Contact,
Educational Meetings, and Community Outreach Programs as Listed:
Leaving a Legacy – Past, Present And
Future
Life review includes reflection on personal stories and mementos.
Professionals guide participants in capturing and sharing part of
their life story during a two-hour program.
Where There’s A Will, There’s A Way
We guide participants in exploring their values and options regarding
end-of-life choices, such as hospice, comfort care, ethical wills,
advance medical directives and creative funeral planning.
Train The Trainer
We provide training for professionals and organizations to create
their programs related to end-of-life issues. One of our programs
is “Facilitating Conversations About End-of-Life”.
